By filling up and submitting this form I am expressing my interest in possibly joining Natural Earth Farms' CSA Program. Once the possible site is confirmed, the information I provided here will be used by Natural Earth Farms to contact me on how to proceed to the next step to participate in the CSA Program. I understand the Natural Earth Farms’ CSA Information. I understand that if I confirm participation, the program entitles me to a weekly share of produce provided by Natural Earth Farms. The weekly share of Certified Organic produce will be provided for the duration indicated. Once I confirm my participation, I agree to the conditions of the program that the farming and crop production has some risks associated with it like weather, disease, and insect problems. Once I confirm my participation, I am responsible for picking up my share directly from the assigned location at a designated schedule. I will agree to pay the membership fee as indicated on the payment terms sent to me by Natural Earth Farms, once the possible CSA Site I am interested to join in is confirmed. I understand that all payments for the CSA Membership is non-refundable.
